An Apollo 15 - Scott Worden Irwin - well worn cloth Baseball Cap with one row of silver thread Scrambled Eggs to mean Commander (see Wikipedia searching on "Pilot Cap Scrambled Eggs") accompanied by a photocopy of a letter from B. G. MacNabb, Director of Launch Operations for General Dynamics.
"I received these caps from the Astronauts on Project Mercury or from one of the Astronauts involved in the multiple crew flights during the Gemini and Apollo programs in appreciation for my part in making their mission successful; and also because of the close personal relations established throughout the programs. Most of them were presented to me in person, but some through the Program Managers."
I do not note any damage other than lots and lots of sweat staining, into and onto the patch on the front of the hat. This could have belonged to any of the three astronauts as follows, as I do not see any markings at all. Excellent condition other than the sweat:
